投標(biāo)人須提供以下文件
The following documents must be provided.
（1）投標(biāo)人須按招標(biāo)文件第一冊第四章格式IV-9.資格證明文件格式的要求填寫和提交有關(guān)資料；
All Bidders are requested to fill in according to the requirements of Format IV-9 in Section IV and provide relevant information；
（2）投標(biāo)人必須是制造商或制造商直接授權(quán)的代理商； 物資產(chǎn)地在中國境內(nèi)，只接受制造商投標(biāo)。
Bidder must be the Manufacturer itself or directly authorized Agent；
（3）如為代理商投標(biāo)：
If as an Agent:
須提供制造商對(duì)本次投標(biāo)出具的唯一正式授權(quán)書；
1)The bidder should provide a letter with original sole official authorized by Manufacturer for the bid；
2)投標(biāo)人在當(dāng)?shù)匾婪ǖ怯涀缘臅r(shí)間距開標(biāo)之日須滿三年，并提供營業(yè)執(zhí)照等注冊登記的有效證件復(fù)印件；
The Agent shall get registered in accordance with the local law for at least three years from the bid opening date and copies of valid registration documents such as business license shall be provided;
3)投標(biāo)人須具有同類產(chǎn)品的銷售業(yè)績，并提供相應(yīng)證明其銷售業(yè)績的合同或訂單復(fù)印件，并由投標(biāo)人代表簽字或蓋章；
The bidder shall have sales performance for similar products. Copies of the contract / purchase order evidencing its sales performance should be provided and signed or sealed by the Bidder;
（4）投標(biāo)人須提供
The Bidder shall provide the original or copy of the credit certificate issued by the bidder's Bank of deposit within three months before the bid opening date (The signature or sealing by the respective of bidders is needed)；
（5）第八章中要求的其他合格的資格證書。
